They’re businesses. They operate to make profits. How do you make profits? By giving away as little, and collecting as much. It’s the opposite of the imperative that we have as the representatives of the people. But this is so utterly predictable because you didn’t vote for Medicare. Your party didn’t vote for Social Security. You had control of the House, the Senate, the Judiciary, the presidency for eight years and you didn’t do anything. No, I shouldn’t say that. You passed Medicare Part D, which said, let’s see if we give them enough money if they’ll do the right thing. Donut hole, anyone? You know, this is like Jabberwocky, which was the language spoken in Alice in Wonderland, isn’t that right Mrs. Baldwin? Through the Looking Glass. You know, if you say these words enough, sooner or later they take on a little life of their own. Well, you can feel free to do that, but the American people know you’re standing in defense of a system that everyone knows is broken, everyone knows is too expensive, everyone knows doesn’t work, and we were elected to change that. An Open Letter To President Obama on Health Care Reform and What It Means To My Family
Mr. President:
I know that you are a very busy man so I will try to keep my questions about health care reform and the recently passed legislation as short and simple as possible.
- I could ask you why you think this is a good piece of legislation even though I truly believe that it will be a failure and will come very close to bankrupting the country. The basis for my conclusion has nothing to do with political partisanship (in fact, I have never voted for a Republican for national office in my life.) From my perspective, “Obama Care” never effectively addressed the root causes of our escalating health care costs: Americans eat too much of the wrong kinds of food, they exercise far too little, they are overweight, they smoke too much, and they are getting older. This legislation does not address these causes, it just raises taxes and moves money around within the bureaucracy. I could ask you about this but I will not.
- I could ask you why you have not stepped forward and denounced those in your party that have likened Americans like myself, i.e. those that have legitimate and honest concerns about this health care reform bill, to the racists who fought against the civil rights movement from the 1960s. I thought that we lived in a free country where citizens could freely address their elected representatives without being slurred in the most debasing way possible, just for having a different opinion. Your lack of fortitude to oppose those Democrats who frequently use the term “racist” to malign myself and those Americans expressing their honest opposition, cheapens the bravery and contributions of those from long ago that fought actual racism. I could ask you about this but I will not.
- I could ask you why you felt it necessary to pass this legislation by the back door called reconciliation. This is a major, major issue in the country that will affect every American for decades to come. Sneaking it in the back door, without using the traditional, time honored method of passing laws in his country, belittles the approach and makes it look like it was forced through without the full weight of the democratic process behind it. I could ask you about this but I will not.
Here is what I will ask you about. But first, some background facts:
- Let me reiterate that both my wife and myself have never voted for a Republican for national office in our lives.
- We both spent several decades of our lives working hard for AT&T, retiring several years ago, secure in our thinking that AT&T’s promise of health care benefits and coverage for our long years of service was a good bet.
- We both try to eat well, we exercise at our local YMCA on an almost daily basis, neither of us smoke, and we rarely drink. In other words, we take personal responsibility for our health and our health care.
One reason for our personal responsibility behavior is that we are on a high deductible insurance plan with AT&T. We are each responsible for the first ,200 of our annual health care costs before we get any insurance coverage at all. However, for this personal responsibility, we also pay nothing in annual premiums.
- During the debate leading up to the passage of health care reform, you reiterated more than once that those of us that currently had health care coverage would be able to keep it. However, in a recent article in Fortune magazine, the CEO of AT&T, Randall Stephenson, was interviewed (several pages of the article are attached). Towards the end of the interview, he was explicitly asked whether AT&T would consider dropping health care insurance coverage for its employees and retirees. His response made it clear that this was a very viable option for two reasons. First, from a business profitability perspective, under the new health care reform law, “you’re better off paying the government a fine and dropping health care coverage for your employees”, improving AT&Ts bottom line. Second, he talks about “economic gravity” which appears to be code words for “if others in his industry do it, AT&T will have no choice but to do it also.”
Thus, a few quick questions for you:
1) Were you just naive when you made the comments that we could all keep our current health care insurance, not realizing the simple fact that companies are in business to make money and if this bill makes it easier for them to make more money by not insuring their workforce, that is what they will do? Or were you being disingenuous, knowing that this would happen and deliberately misinforming the country to help get your health care reform bill passed? Naive or disingenuous, in either case you will be making millions of American voters unhappy in November and in 2012 when we are forced out of our current health care coverage and will blame you for either ignorance or arrogance in this situation.
2) I am 57 years old and my wife is 56 years old and if Mr. Stephenson does decide to terminate AT&T’s health care coverage for employees and retirees, where do you suggest that my wife and I get coverage? What insurance company is going to want to pick us up, and millions of other older Americans who lost their coverage, at our ages even though we are both healthy and taking personal responsibility for our continued good health?
3) If we are forced out onto the market for health care insurance coverage, our new coverage is likely going to be much more expensive. Our annual health care costs will go from a maximum of ,200 each to a minimum of several thousand dollars each. Is this how you planned to reduce health care costs for middle class America? Is so, then you need to explain the math to me. Maximum of ,200 to a minimum of several thousand dollars, does not make sense out here in the real world. How does this reduce the escalating health care costs for the 90% of Americans that already had health care insurance prior to the passage of this bill?
Thus, I am not going to ask you about why you and the rest of Congress did not address the root causes of high health care costs in your legislating process. I am not going to ask why you have sat back and been silent while those Americans with legitimate and honest dissent against this bill have been likened to racists by members of your party. I thought you represented all Americans, not just those that agreed with your policies. I will not ask you about why you did not have the courage and guts to pass this legislation the right way, through the front door like every other piece of legislation, but instead snuck it through the back door of reconciliation.
However, I will ask you or your staff to contact me and explain where and how I can get health care coverage at my age if AT&T and the rest of corporate America decides it is a better economic choice to pay a government fine than to cover their employees and retirees with health insurance. I will ask you to explain whether you were naive or disingenuous when explaining that we would be able to keep our current health insurance coverage. And finally, please explain how paying no more than ,200 a year under my current coverage (with many years paying nothing for coverage during healthy years) is a better deal then finding new coverage at my age and paying several thousand dollars a year for the privilege.
Although I have written to the White House many times, I have never received any answer to my questions on a wide variety of topics even though you promised to have the most open and responsive administration of all time. That has not happened yet. However, in this case I do require, in fact I demand specific answers to my three questions above. For your political sake I hope to receive those answers before early November and certainly before 2012.

Advance Health Care Directives And Living Wills: A Step-By-Step Guide
Few decisions are more personal — involving both health and death — than those embodied in an advance health care directive or a living will, or a similar document. Some individuals want their lives prolonged by any means necessary, while others want medical treatments withheld, allowing for a natural death.
An advance health care directive lets caregivers and family and medical providers know a person’s healthcare wishes if he’s unable to speak for himself. The document can also appoint someone else to speak with legal authority for the person if he’s unable to do so for himself.
If someone plans to set up an advance health care directive, living will, or similar document, here’s what he needs to think about:

1. The person should consider how he wants to live during a terminal illness, and what his end-of-life preferences are when creating a health care directive or living will.
It’s not easy to bring up the subject of dying (especially if it’s your parent you’re caring for). But you, the individual, and the rest of his family will gain some peace of mind if you can get him to start thinking and talking about this subject, and eventually to execute documents that set down his wishes.
·One way many people get this discussion started is by showing the person the advance health care documents they’ve prepared for themselves. This both breaks the ice and gives the person a model on which to base his own documents.
·The job is to get him thinking about what will be important to him when he is incapacitated, and particularly when he’s dying, such as where he wants to be — at home if possible, or in a hospital — and what treatments (particularly life-prolonging ones) he wants and doesn’t want.
·He also needs to think about who he wants to make decisions for him if and when he’s unable to do so himself.
·The person should discuss these things with family, healthcare providers, and trusted advisors who will help him consider his wishes, options, and fears.
·But remember that a conversation doesn’t have the same legal force as an advance health care directive or similar written document, even if a doctor records it. In most states, only a written, signed, and witnessed advance health care directive legally must be followed by healthcare personnel and institutions.
2. The person should carefully consider whom he wants to serve as the agent to make decisions for him and support his choices when creating a health care directive or living will.
The agent the person in your care names in his advance health care directive should have several qualifications:
·The job can be emotionally difficult, so it should go to someone who cares deeply about his welfare.
·It should also be someone who is likely to be able to remain physically near him during a prolonged healthcare crisis.
·And it should be someone who has a strong enough personality to stand up to family members, doctors, and hospital personnel if necessary.
·Sometimes a trusted friend will make more objective decisions, or will follow the person’s wishes more carefully, than a family member.
Whomever he winds up choosing, he has to discuss the responsibility with that person and make sure she’s willing to accept it before naming her in the document.
3. Use sample forms for the advance health care directive or living will as planning tools.
Situations to address
Each individual’s advance health care directive should be personalized to reflect his particular wishes. An advance health care directive should address situations such as:
·When (if ever) the person in your care would want artificial life-sustaining treatment, such as during permanent unconsciousness or severe dementia
·Types of life-sustaining treatment he would and would not want, such as artificial nutrition and hydration, surgical procedures, and cardiopulmonary resuscitation (CPR) — and under what conditions
·Instructions about other medical procedures that may arise given the person’s medical history
·Organ donation instructions
·Pain control preferences
·Where he wants his care (at home or at a designated nursing facility, for example)
Documents available online
Generic advance health care document forms are available from many sources, but the forms are designed to be planning tools only. They don’t offer a final product — the forms are meant to trigger but not replace communication between the person in your care (the principal) and the designated decision maker (the agent).
Still, they are useful for getting started. Most state legislatures provide official forms for advance health care directives and living wills. These examples, among others, are available online:
· New York
· California
· Delaware
· Illinois
· Oregon
· Massachusetts
Similar forms are provided by such groups as state medical and bar associations. For example, sample forms can be found online through the California Medical Association, AARP, and the American Medical Association.
The importance of state forms
·Though there’s no single form that must be used for an advance health care directive, an individual should use his state’s standard form if it has one. He should also follow his state’s signature and witness requirements. For example, most states require two witnesses to the person’s signature; some states also require notarizing the document.
·In most states, witnesses cannot include relatives, heirs, medical providers or their employees, or anyone responsible for the patient’s healthcare costs. If the person in your care is in a nursing home, some states require a state nursing home ombudsman or patient advocate to witness the signing.
·Once the document is executed, the individual should give copies to his doctors and hospital, the person he designates as his healthcare agent, family members, and other advisors. He should keep a list of individuals and institutions that have a copy of the advance health care directive, in case he ever wants to revoke or change it.
4. Update the advance health care directive or living will based on changing end-of-life wishes.
·Encourage the person you’re caring for to revisit his advance health care directive periodically, as long as he’s able to do so. His feelings and choices may evolve as his situation changes — for example, after he’s diagnosed with a serious illness, or as he witnesses others going through the end of life.
·People commonly make changes right before a major surgery. He may also change his feelings about who should act as his healthcare agent, or his original choice might no longer be able to take on the job.
·He can terminate or change the advance health care directive as long as he has the mental capacity to do so. If he wants to make a change, he should prepare and sign a new document and have it properly witnessed again; just making handwritten changes on a previous document is not a good idea.
·If he does execute a new document, he should send a copy to every person and institution that has a copy of the previous one, explaining that the new one replaces the old. In most cases, completing a new advance health care directive automatically revokes all previous directives, but it’s always a good idea to let everyone know personally.

If you are travelling abroad, you should take out travel health or medical insurance to help you cover your medical expenses in case of an emergency abroad. Note that medical costs and hospital care abroad can be steep and US-issued policies are generally not accepted outside the US. Medicaid and Medicare programs also do not provide coverage for medical or hospital expenses incurred outside the US.
This type of short-term insurance policy is specifically designed to provide temporary medical coverage for periods as short as five days to as long as a year, and is renewable for two to three years. Major medical insurance policies are also available for those who are travelling abroad for longer periods of time such as six months to a year. Apart from medical care, travel insurance also covers evacuation, accidental death, trip interruption and limited baggage. If you have a major medical policy, however, you have more comprehensive coverage that includes prescription drugs and wellness care.
The types of travel insurance that you can avail of include comprehensive, emergency and medical evacuation. Comprehensive temporary insurance is for those who have no insurance at all, even at home, because they’re between jobs or living and working overseas. Emergency health insurance, on the other hand, is for those who do have coverage but need a supplemental policy to cover the costs of emergency medical care abroad that are not included in their regular policies. If you are so badly hurt that you cannot travel on a regular commercial flight, a medical evacuation (medevac) policy will pay for costs such as an air ambulance and doctors’ and nurses’ fees; however, hazardous activities such as bungee jumping or extreme sports may be excluded from coverage. Other kinds of travel insurance cover the costs of trip cancellation for any reason, including illness and injury of the insured, a travelling companion or an immediate family member, and theft or damage of a vehicle that you have rented.
You can get travel insurance from some travel agencies or suppliers, who include coverage as part of the cost of the ticket. Your regular health insurance provider may also be able to provide you with a supplemental policy that includes travel insurance. There are also providers, such as travel insurance companies and direct providers of medical evacuation and emergency services, who specialize in this type of policy.
To find the best travel insurance, make sure that you are clear on exactly what coverage you need to prevent you from paying extra for something that you will not use. Then do a general search of the different short-term insurance providers and then compare quotes. But when you are asking for quotes, avoid giving out any personal information that is not directly related to your trip in order to avoid getting scammed. Apart from the quotes, also look at just what the extent of the coverage each company provides will be, particularly if you are bringing along expensive electronic devices such as a laptop or camera; make sure that the plan either reimburses the loss of these or that you can specify that you want them covered.
